The vision care program provides for full-time employees and their eligible dependents to receive a $80 payment for prescription single-vision eyeglasses or contact lenses and a $90 payment for prescription progressive/bifocal/trifocal eyeglasses or contact lenses. Full time employees and eligible dependents are also eligible for a maximum payment of $45 or the cost, whichever is less, of an eye examination by an Ophthalmologist or an Optometrist. Dependents are eligible until the end of the year of their 26th birthday. Proper affidavit and submission of itemized receipts in Workday are required of the employee in order to receive payment. Each eligible employee and dependent may receive only one payment for glasses/contacts and one payment for examinations every two year period as determined by Union negotiation. The current reimbursement period is July 1, 2023 through June 30, 2025. To receive reimbursement please complete the following steps: 1. Complete this form Vision Care Program Reimbursement Request. Save the completed form to your device 2. Log into Workday and access your profile. Click on the Actions>Benefits>Change Benefits>Vision Care Program Reimbursement Request tasks from your profile. 3. Enter the information requested from Workday and follow the prompts to upload the form with itemized receipts.
